Provider Disclosure:

Services may be provided in this health care facility by the facility as well as by other health care providers who may separately bill the patient and who may or may not participate with the same health insurers or health maintenance organizations as the facility.

If Bayview Surgery Center believes that you have health insurance and/or HMO coverage(s) that may cover some or all of the Services, Bayview Surgery Center may initiate contact to determine your cost-sharing responsibilities for the facility portion of your bill. You may contact them directly as well for additional information concerning your cost-sharing responsibilities.

Good Faith Estimate:
Patients and prospective patients may request from this facility and other health care providers a more personalized estimate of charges and other information. These will be only a non-personalized estimate of costs based on the scheduled or anticipated services and actual cost may vary based on actual provided services. Patients and prospective patients should contact each health care practitioner who will provide services in the ASC to determine the health insurers and health maintenance organizations with which the health care practitioner participates as a network provider or preferred provider. You may receive a good faith estimate by contacting Bayview Surgery Center at (941) 373-9808. (Practitioners may not participate with the same health insurers or health maintenance organizations as this ASC.)

Itemized Bills:

All patients will receive an easy to understand, all-inclusive bill for services provided at the center. Patients could prospectively receive itemized bills from other providers who provide services in the surgery center as part of the patient care.

AHCA Pricing Information:

Service bundle information is a non-personalized estimate of costs that may be incurred by the patient for anticipated services and actual costs will be based on services actually provided.

Insurance Information:

Bayview Surgery Center accepts Medicare, Medicaid and most commercial insurance plans.

Specifically, Bayview Surgery Center has written agreements with the following insurance plans:

Charity Care:

If Bayview Surgery Center determines that you have cost-sharing responsibilities for the facility portion of your bill, in accordance with Bayview Surgery Center’s financial assistance policies, you will be required to pay your cost-sharing responsibilities in full on the date that Services are provided. Bayview Surgery Center’s financial assistance policies are that if you are unable to pay your cost-sharing responsibilities in full on the date that Services are provided, because you believe you are medically indigent or you are not covered by any health insurance or HMO, then upon request Bayview Surgery Center, in its sole discretion, may offer you a payment plan. There is no formal application process for obtaining “charity care” at Bayview Surgery Center.

Self Pay/Uninsured Patients:

We also welcome self-pay and uninsured patients. Payment in full or an arranged payment plan will be expected prior to the day of your procedure. Bayview Surgery Center offers both payment plans and financing options.

No Surprises Act Information:

The No Surprises Act protects patients covered under group or individual insurance plans from receiving surprise medical bills for most emergency services and non-emergency services from out-of-network providers at in-network facilities. Beginning in 2022 there are new protections that prevent these surprise medical bills.
